Prayer Request Form

If you are a member of Cornerstone Church, you should post prayer requests on Cornerstone Connect. Non-members may submit a prayer request by calling the church office at (336) 665-1944, or using the following online form.

    The name, phone, and email fields are optional and are only used in case the Prayer Ministry Team needs to contact you for clarification. This information is not included in the prayer request.
